Borrowing the Sample Disk: How to fill out the Request Form

  1. Complete and submit the online Lunar and Meteorite Sample Disk Request Form:

    Remember:
    • Submit your request at least SIX WEEKS in advance
    • Be sure to include 3 optional dates to borrow the Sample Disks
    • Sample Disks can be borrowed for up to two weeks at a time
    •  
  2. Once your information is processed and your request dates are finalized, you will be emailed 2 forms: 1) Loan Agreement & 2) Security Form.
    You must:
    1. Print a copy of each form
    2. Complete and sign each form:
      • You will need a supervisor signature as well as your own
      • Be sure to list ALL places where the disks will be used
    3. Make a copy of the signed documents (for your records)
    4. Mail the Loan Agreement and Security Form (with the original signatures) to the address provided on the documents, along with a copy of your NASA Lunar and Meteorite Sample Disk Certification
