LMDP Request Form Information

In order to request the Lunar and Meteorite Disk, Please fill out the following PDF form and click submit when you are finished.

Here are a few things you MUST KNOW when filling out this form:

  1. When filling out the phone number, use numbers only. DO NOT use spaces, parenthesis or dashes. The number will automatically format itself when you tab to the next field.
  2. You may need to temporarily allow pop-ups on your computer.
  3. You may need to choose the correct email carrier.
  4. You SHOULD receive a confirmation email back within a day or two of submitting your form. If not, please use the contact information below to send a follow-up email to make sure your request form didn't have an error during the submit process.

Once I receive your information back, I will forward you a loan agreement and security form to complete, sign and mail back to me. If you have any questions or problems with this process, contact:
